Scope of Coverage
Vitable Health is a direct primary care provider focused on overall health management, prevention, diagnosis, and treatment of common illnesses and conditions. We primarily deliver care through telemedicine, with in-home physicals and blood pressure checks available in certain regions. Your care team includes nurse practitioners and doctors.
Vitable Health offers various membership plans, which may vary based on your employer. This article outlines the scope of services and coverage for our plans.
Vitable Primary Care - Core
Annual Wellness Visit (1 per person per year, telemedicine) – Establishes care with a Vitable Health provider, assesses baseline health, and includes standard lab orders.
Unlimited Primary Care Telemedicine Visits – Covers chronic condition management, prescription refills, acute illnesses, sexual health, and more.
Primary Care Lab Work – Includes standard primary care tests and STI testing through Labcorp (must be provider-ordered). See covered labs here.
Medication Benefit – Covers provider-prescribed medications on the formulary (no controlled substances and MOST injectables). Show your virtual pharmacy card for coverage. Discounts available for non-formulary prescriptions.
Mental Health Coaching – Unlimited virtual talk therapy visits for members and dependents 18+.
Flu Vaccine Coverage – Seasonal vouchers available (September – March) upon request.
Household Coverage – Dependents (18 months–64 years) can be added at no extra cost, regardless if they’re related to you. Members 18+ need to download the Vitable Health app to their own account.
Vitable Primary Care - Enhanced
Annual Wellness Visit (1 per person per year, telemedicine) – Establishes care with a Vitable Health provider, assesses baseline health, and includes standard lab orders.
Annual Physical Exam (1 per person each year - In-home visit). Vitable Health will send a provider out to your home.
Please note that availability for this service varies by region.

This plan includes all benefits from Vitable Primary Care – Core or Enhanced, plus:
Preventive Care Coverage – Get reimbursed for preventive services at a provider of your choice.
Reimbursement Model – Pay out-of-pocket, submit an itemized invoice, and receive reimbursement directly to your bank (no insurance cards needed).
Covered Preventive Services – Includes annual physicals, screenings, immunizations, well-woman visits, birth control, and cancer screenings. Full list here.
Children’s Preventive Care – Covers well-child visits, vision/hearing screenings, and vaccines (see attached document for details).
Claim Submission – Follow these steps to submit a claim.
MEC Plus
This plan includes all benefits from Vitable Primary Care – Core or Enhanced, plus:
ID Card – Available in the Vitable App.
Preventive/Wellness Care – Access services from providers outside Vitable Health.

Office Visit Coverage (for members using in-network providers):
Primary Doctor/Pediatric Sick Visits – $15 co-pay, $200 limit per visit, max 5 visits per year.
Specialist Visits – $25 co-pay, $200 limit per visit, max 2 visits per year (no coverage for in-office procedures).
X-Rays – $50 co-pay, $200 limit per visit, max 3 visits per year.
Urgent Care – $25 co-pay, $200 limit per visit, max 2 visits per year.
