Step 1: Locate a Quest Diagnostics Facility Near You

  1. Go to the Quest Diagnostics Appointment Locator.

  2. Enter your ZIP code, city, or state to search for Quest facilities near you.

  3. You can also use filters to sort by specialty or services offered.

Step 2: Choose Your Preferred Location

  • Review the list of available Quest locations.

  • Select the most convenient location based on proximity, services offered, and hours of operation.

Step 3: Schedule Your Appointment

  1. On the Quest Diagnostics website, click "Schedule an Appointment" for the selected location.

  2. Sign in or create an account if you don’t have one yet. (You can use an existing account if you’ve been to Quest Diagnostics before.)

    • If you already have an account, simply log in with your username and password.

    • If you don't have an account, you'll need to create one by providing your basic details like name, date of birth, contact information, and insurance details (if applicable).

Step 4: Enter Your Appointment Information

  1. Select your tests: Quest Diagnostics will ask you to choose the test(s) you need to schedule. These may be provided to you by your healthcare provider or specified by a voucher (e.g., Vitable Health). If you don’t know which test to schedule, Quest may allow you to search by test type.

  2. Choose appointment date and time: Once you’ve selected your tests, you’ll be prompted to choose a date and time that works for you. Quest Diagnostics will show available slots based on your location and their availability.

Step 5: Confirm Your Appointment

  1. Review your appointment details to ensure everything is correct (tests selected, date, time, location).

  2. Click on “Confirm Appointment” to finalize your booking.

Step 6: Receive Confirmation

  • After confirming your appointment, Quest Diagnostics will send you a confirmation email or text message with your appointment details. This will include the location, date, time, and any instructions related to the tests you’ve scheduled.

  • You may also receive a QR code or confirmation number to bring with you to your appointment.

Prepare for Your Appointment

  • Quest Diagnostics may provide specific instructions for your tests, such as whether you need to fast beforehand or other preparations.

  • Make sure to bring your voucher or lab order (if applicable), insurance information, and a valid ID to your appointment.

Attend Your Appointment

  • On the day of your appointment, arrive at the selected Quest Diagnostics facility at the scheduled time.

  • Present your voucher, insurance information, and ID at check-in.
