If you have recently lost access to your benefits due to termination from your employer, you may qualify for COBRA continuation coverage. Below is a simple guide on what to expect and how to extend your benefits through COBRA.

What Happens After Termination

  1. COBRA Election Notice Sent:

    • Once Imagine360 notifies UnifyHR of your termination, UnifyHR will mail a COBRA election notice to your last known address.

    • This notice is typically sent within 3-4 days after UnifyHR is notified.

  2. What’s Included in the COBRA Election Notice:

    • Instructions on how to elect coverage and the associated costs.

    • Your assigned member number from UnifyHR.

    • Contact information for UnifyHR.

    • Steps to make an election online.

    • Payment options for your monthly premium.

How to Reinstate and Maintain Coverage

  • Reinstating Coverage:

    • After you make your first payment, UnifyHR sends a report to Imagine360, which reinstates your coverage retroactively to your termination date.

  • Making Monthly Payments:

    • Payments can be made via the following options:

      1. Online on the UnifyHR website.

      2. Monthly ACH (automatic bank transfer).

      3. By mailing a check each month.

  • Continuing Coverage:

    • As long as payments are made on time, your COBRA coverage remains active until:

      • The COBRA period ends.

      • A premium payment is missed.

      • You request to terminate the coverage.

Financial Processing

  • UnifyHR deposits the payments collected into the Vitable bank account and sends a remittance report for tracking purposes.
