Will I receive a Form 1095 A?

You will NOT receive a Form 1095-A through Vitable Health. This form is only issued if your health insurance was purchased through the Health Insurance Marketplace (also known as the Exchange), which does not apply to Vitable Health plans.

What Will I Receive Instead?

If you are enrolled in a MEC, MEC Plus, or MVP plan through Vitable Health, here’s what you will receive instead:

  • Form 1095-C: If you are covered by a large employer's health plan (generally for employers with 50 or more full-time employees), you may receive this form. It provides information about your employer’s health coverage and whether it meets the Affordable Care Act's (ACA) requirements.

The forms will be filed on March 31, and employees should expect to receive them sometime after that date. If you have not received your 1095-C, please contact your employer directly for an update.

What To Do If You Need a 1095-A?

If you received coverage through the Health Insurance Marketplace and believe you should receive a 1095-A, you’ll need to contact the Marketplace where you enrolled (e.g., HealthCare.gov or your state’s exchange) to obtain the form.
