Overview

Create custom forms for customers to complete and waivers to sign upon selecting a service type. You may create unlimited forms and waivers for your organization and multiple waivers/forms may be attached to a single service type. Completing waivers and forms may be made optional or mandatory for the customer.

Create a New Waiver or Form

  1. Select the Add Waiver button at the top right of the of the page

  2. Enter a title and description (instructions) for the form. You may also choose to include a signature field and specify which locations may have access to the form.

  3. Select Save button then the Add Question button at the bottom of the page. This will open the Question Configuration Window.

  4. Choose the answer field type for this question. There are several options including text field, number field, date wheel, checkboxes, radio buttons, and more.

  5. Choose whether this question is optional or mandatory

  6. Enter a question for the customer to answer

  7. Enter an optional short name for this question for reporting purposes

  8. Choose conditions upon which this question is visible to the customer. For example, you may choose to only show a question if the previous question was answered in a particular way.

  9. Repeat this process for as many questions as you would like to have in the form.
