The information provided below is for either the Transfer from Decedent to Executor or Administrator in their fiduciary capacity or Transfer from Decedent to a Transferee other than the fiduciary.

Documents needed to register the stock into the name of an estate:

  1. Court certified document naming estate representative

  2. A copy of the death certificate

  3. An original Affidavit of Domicile (.pdf link or similar)

  4. Physical Stock Certificate (if applicable)

With these documents we are able to register the certificate(s)/book-entry position(s) into the name of the administrator for the benefit of the estate, ex. Jane Doe Personal Representative FBO Estate of John Doe.

Additional Document needed to transfer the stock out an estate:

  1. An original Stock Transfer Form with proper endorsement by the executor or administrator of the estate with Medallion Signature Stamp Guarantee affixed

IMPORTANT: The Affidavit of Domicile requires a notary.
