What is a challenge survey?

Gather valuable feedback and insights from your challenge participants by running a brief survey before and/or after your challenge using challenge surveys. Users will respond to and submit their surveys from within the Wellbeing+ app.

Pre challenge surveys are required to submit before a user is able to join the challenge. Post challenge surveys appear after a challenge ends and are required to submit in order for the user to view final challenge stats.

Challenge surveys can be used for a number of reasons, including:

  • getting a pulse on participants’ perception of their wellbeing

  • collecting challenge experience feedback

  • gathering insights on user challenge preferences

  • gauging user interest in future challenge initiatives

Surveys are intended to be short, interactive, and easy to navigate so that users can provide feedback during their challenge experience in a seamless way.

We've put together several example survey templates to help get you started. You can find them here.

Survey question types

There are a variety of interactive question types that you can use to craft your challenge survey:

  1. Single Select - Users select one or more options from a list of predefined options. Only one selection is allowed.

  2. Checkboxes - Users select one or more options from a list of predefined options. Multiple selections are allowed.

  3. Star Rating - Users submit a rating using a 5-star scale.

  4. Number Rating - Users submit a numerical rating. You can choose to label the minimum & maximum value options, or label each individual value.

  5. Short Answer - Users submit short text-based feedback.

Challenge visibility & inviting users to join challenges with surveys

There are a few things to be aware of when you enable surveys on a challenge:

  • When you enable challenge surveys:

    • Your challenge visibility will automatically be set to hidden to ensure no users join your challenge before you are able to create your survey(s). Once you've created your survey(s), you will be able to set the challenge to public if desired.

    • You will not be able to access the challenge invitation link or in-app invites until after you create your survey(s). You need to set up your survey(s) before you invite users to join the challenge.

  • You cannot manually add participants into a challenge that contains a survey. You will need to either make your challenge public so that your users can find and join the challenge themselves, or you will need to invite them to join the challenge using an in-app invite or by sharing the challenge invitation link.

  • Once you invite users to join the challenge or set the challenge to public, you will no longer be able to make edits to, delete, or add new surveys.

How to set up a challenge survey

  1. Select the Challenges tab in the side navigation menu of the Admin Center, and begin the process of creating your challenge. In step 1 (Challenge Details), toggle ‘Set up your challenge surveys’ to on. Proceed through the remainder of the challenge creation process.

    • If you initially created a challenge with surveys turned off but now wish to enable them, you can do this by clicking on Edit Details in the challenge dashboard and toggling surveys on.*

    • *You will only be able to turn on surveys if you haven’t already invited users to your challenge. If participants have been invited to or joined the challenge, the option to turn on surveys will not be accessible.

  2. Upon finishing challenge creation, you’ll see a confirmation modal which navigates you to the surveys tab for set up. Select Go to Surveys Tab to start creating your survey(s), or select Later to navigate to the challenge dashboard if you’ll be completing this task at another time.* You can access the Surveys tab from the challenge dashboard under the "Tasks to Complete" section.

*Please note that you are required to set up your challenge survey(s) before you’ll be able to invite users to your challenge.

  3. From the Survey Set Up screen, locate the survey type that you want to create, and select Add Survey to access the survey builder.

  4. On the survey builder screen, you can start building your custom survey experience. To start adding questions, reference the Question Type container on the right-hand side.

    • You can click the arrow to expand each question type for additional information and for a preview of how the question will appear in the mobile app.

    • Click on the blue plus sign to add this question type to your survey.

  5. Continue to add questions and associated answers until you are satisfied with your survey. You can rearrange the order of your questions by clicking and dragging the six dot icon. Please note that you can have a maximum of 7 questions per survey.

  6. Select Save & Finish.

  7. You should now see that you have a survey set up under the survey type that you’ve selected. You’ll be able to make edits by selecting Edit Survey, or delete the survey from your challenge if needed by selecting Delete Survey.

*Please note, you can only edit or delete surveys if you haven’t invited any participants to the challenge. Once you have invited participants you will no longer see the option to edit or delete surveys.

  8. Repeat steps 3-7 if you’d like to create another survey type. You only need to create one survey for a challenge to proceed to inviting users to your challenge. If you’re all set, select the Go to challenge dashboard button.

How to view your challenge survey

Before you’ve invited users

Select the Surveys tab to view, edit, or delete any surveys you’ve set up. You can also add additional surveys at this time if you’d like.

After you’ve invited users

Once users have been invited to or joined the challenge, you will no longer be able to make edits to, delete, or add new surveys. Upon selecting the Surveys tab, you’ll be able to see the active survey by clicking View Survey.

Viewing Survey Results

You can view the results of your survey by pulling a Survey Report from the Reporting tab in the Admin Center. We recommend allowing at least one week after your challenge begins or ends to pull a survey report so that you ensure you capture as many submissions as possible. If you run the reports too early, you risk missing submissions from all participants, which could potentially skew the data or not capture the full picture.

Follow the steps below to pull a Survey Report:

  1. Go to the Reporting tab in the Admin Center

  2. Locate the Survey Report and select the Run Survey Report button

  3. In the overlay, select the relevant challenge under Filters

  4. Select the Run Report button

The Survey Report will display basic user information as well as each question you’ve set up for your survey. Each user that completed the survey will have an answer listed in the question column.

FAQs

Can I reuse a challenge survey?

  • You need to manually create each survey from within its associated challenge dashboard. If you wish to reuse the same questions from a previous survey, you can access the previous survey questions by going to the survey tab in the challenge dashboard. You can then copy and paste those questions into your new survey, or save them in a personal document for later access.

Are challenge surveys mandatory for users to fill out?

  • Yes. If you’ve set up surveys for a challenge, then users will be required to fill out the survey in order to fully participate in your challenge. If a user does not fill out a pre challenge survey, they will not be added into the challenge. If a user does not fill out a post challenge survey, they will not be able to see final challenge scores.

Are challenge surveys anonymous?

  • No. Upon pulling a Survey Report, survey responses will be visible for each user.

Do I need both a pre and post challenge survey for a given challenge?

  • No, only one survey type is required if you’ve enabled surveys for a particular challenge. You can choose to run just a pre-challenge survey, just a post-challenge survey, or both.

Should I run a survey for every challenge?

  • How often you choose to run challenge surveys will depend on what you are hoping to achieve. You can include a survey in every challenge, or space them out quarterly, annually, or on whatever cadence makes sense for your program. Regardless of your chosen frequency, it's important to keep surveys concise and to regularly review the collected feedback. Use the insights gained to take relevant actions, ensuring that each survey remains impactful and valuable for both you and your users.
