How to View the Pre-Assessment or Intake Form Filled By Clients

Reviewing a client’s intake form is an important step in preparing for sessions. The intake contains key information provided by the client during registration, including details that may highlight areas of concern.


Accessing the Intake Form

  1. Log in to the Wellnite Dashboard.

  2. Navigate to the Members tab.

  3. Select the client’s name from the list.

  4. Inside the client’s chart, open the Intake section.

  5. Here, you will find all information completed at registration under Therapy Data.


Important Notes on Intake Submission

  • Clients are required to complete the intake form at least 24 hours before their appointment.

  • If the intake form is not completed, the session may need to be postponed.

  • The admin team is responsible for sending reminders and ensuring clients complete the form on time.


Shared Files

Any additional files exchanged between you and the client are stored in the Shared Files folder, accessible via the files folder.


✅ Reviewing the intake form before each session ensures you are aware of the client’s background, goals, and any red flags, such as suicidal ideation, supporting a smoother and safer therapy experience.
