At Wellnite, accessing and reviewing client questionnaires is an essential step to ensure a smooth therapy session and to identify potential concerns such as suicidal ideation.
Therapy Questionnaire
First, click on the 'Members' tab in your Wellnite Help Desk account.
Locate the specific client whose questionnaire you wish to view and click on 'View' next to their name.
Once you're on the client's profile page, click on 'Clinical Information.'
Under 'Clinical Information,' you will find the option for 'Therapy.' Click on it to access the Member Therapy data, including the Initial Questionnaire.
Important Note on Questionnaire Submission
Clients are required to complete the questionnaire at least 24 hours before their appointment.
Failure to fill out the questionnaire may result in the session being postponed.
The responsibility of sending reminders to clients and ensuring timely completion of the questionnaire lies with the admin team.
Progress (Shared Files)
The 'Shared Files' folder stores any files exchanged between you and your client via the HIPAA-compliant message inbox.
