Evaluation Referral Form

This article shows you where to find and complete an online referral form for special services for children 3-5 years of age.

You can find the online referral form on the WIU webpage. www.wiu7.org

  1. Under SERVICES - choose student

  2. Click on: Early Childhood Education

  3. Click on: How can I refer my child for an evaluation.

  4. Click on: here (This will take you to the online form.)

  5. Click on: GREEN Referral Button

  6. In the drop down box - choose Westmoreland Intermediate Unit

    Please complete the form to the best of your ability, including your concerns in the concerns section. Mr. Collins Carlin will review the form and contact you with more information.
