CM Hub: Submitting Chapter Formation Documents

Prior to signing your Chapter Charter with YPO submit your formation document information.

Usage

Your chapter formation document is the document filed in your jurisdiction that establishes your chapter as a recognized formal legal entity. Submitting this information to YPO allows us to use this legal information in your Chapter Charter with YPO.

Steps

Navigate to Upload Operational Document

1. On the Operations module of the Chapter Management Hub, click on Actions and select Upload Operational Document.

Complete Formation Document fields

2. In the pop-up window click on Document Type and select Formation Document.

3. Complete the required fields:

  • Legal Name (ex: YPO South Chicago Chapter, Inc.)

  • Jurisdiction of Incorporation (ex: Illinois)

  • Incorporation Date

  • Type of Legal Entity (Non-Profit Corporation)

4. Click Upload Files to upload an image, PDF, etc. of your chapter's formation document. Click Done.

4. Click Submit.

The formation document information entered is submitted to YPO for review.

You will receive notice via email when your submission is approved or if additional follow-up is needed.

Locate your Formation Document submission

1. Click into the Operations module on the Chapter Management Hub.

2. Click on Files and Documents.

3. In the table, locate the document title of "Formation Document." The status will be:

  • Pending Review - upon submission until reviewed by YPO management team.

  • Accepted - when accepted by YPO management team.

  • Sent Back - when YPO management team requires additional or clarified information.
