(If the Pharmacy needs to change to a Home form, or encounters an issue in Pharmacy environment, log into Home facility)
Navigate to the Cases module > locate the case within the CM Queue (or Linked Events if it was previously a Pharmacy Incident) (or use the Search button to Search for the Event ID).
For Pharmacy Medication Incidents:
For Home Medication Events (Incident and Good Catch):
On the incident’s Summary & Root Cause Analysis page, select Edit icon to edit what was reported
At the top of the form, select the new event type from the drop down
Save as Draft
Can use Comment section to indicate why this has been done
NOTE: If toggling between a Home event and a Pharmacy event, or vice versa, please alert the other party, either by including a Comment, or by changing the Event Stage to “Form change to Home/Pharmacy” (if set up) to trigger a notification
