Under WHAT Happened, the problem that caused or could have caused harm is that the order was omitted, therefore category is Omission and the subcategory is Request not filled/missing (e.g. item omitted from bag/ package, order not entered/processed)
Contributing Factors for both/either scenario above:
If categories do not fit exactly, free text "Describe the facts of what happened" / “Other relevant facts” boxes at beginning and end of form allow as much free form text as is needed
