When creating membership plans, you can make use of form fields to collect important details about your customers.
If you want to make use of these, in the configuration page, click Add Field at the bottom.
Use the dropdown to the right of each field to select your form field type. Each option will be briefly outlined below:
Text Field
Text field can help you get additional information from your customers if needed. Text provides your customers a textbox they can type notes into.
For example, if they require their products be left at a specific location, such as their porch or doorstep, they can add that before purchasing.
Dropdown
Options will be presented in a dropdown list on your product page for customers to choose from. You configure what the options are, separating each with commas.
The example below is for selecting the age range of customers.
Checkbox
Options will be presented as checkboxes for customers to select.
The example below is for toggling the membership product purchase as a gift (purchasing on behalf of a friend/family member using their payment details).
Radio Groups
Options will be presented as radio buttons, of which only 1 selection can be made.
The example below is the same age range of customers as the dropdown option.
Date Picker
You can set a date picker interface to let your customers pick their subscription renewal date. Renewal is worth emphasizing, as it will not affect the initial order date, which is always on the day of purchase.
Put a name in Field Label, which will be visible on your product pages, enter your country’s date format, then enable specific days of the week.
Note: For any of these 3 options, you can require your customers to interact with the advanced fields with the first checkbox and make it visible on your product pages with the second. Save when you are finished.
You can check your membership product page to view how everything will look from your customers' end.

Below is the checkout page of a sample Shopify order with a form field set (text field) and filled out prior to purchase.
Attributes
When a purchase is made with these fields filled out, they are stored as attributes in both Shopify and Appstle Memberships.
In Shopify, it will be visible in the Orders section. Click on the ID of an order to view the attributes (example is of Delivery Instructions).
In Appstle Memberships, it will be visible in the Memberships section. Click on the Membership ID of a customer to be taken to the Merchant Portal.
From the portal, select the 3-dots icon on the far right of the membership product and select Attributes.
In addition, you can navigate back out to Memberships and Export your memberships (upper-right corner) in a csv file that you can open in either Excel or Google Sheets. There will be a dedicated field for the form field(s) that you can view, as shown in the sample data sheet below:
