When creating subscription plans, you can make use of form fields to collect important details about your customers.

If you want to make use of these, in the configuration page, click Add Field at the bottom.

Use the field type dropdown in each field to select the type. Each option will be briefly outlined below:

Text/Text Area

Text/Text area can help you get additional information from your customers if needed. Text provides your customers a textbox they can type notes into.

For example, if they require their products be left at a specific location, such as their porch or doorstep, they can add that before purchasing.

This shorter message will suit text better...

...whereas text area, providing a larger text box, will suit lengthier, more detailed notes better.

Select

Selection options will be presented to your customers to choose from.

The example below is for age ranges.

Checkbox

Options will be presented as checkboxes for customers to select.

The example below is for toggling the subscription product purchase as a gift (purchasing on behalf of a friend/family member using their payment details).

Order Date

You can set a order date interface to let your customers pick their subscription renewal date. Renewal is worth emphasizing, as it will not affect the initial order date, which is always on the day of purchase.

Type in a name, which will be visible on your product pages, enter your country’s date format, then enable specific days of the week. If you want, you can also set a minimum threshold day for the next order, which functions exactly like regular cutoff days. So for an order to qualify for the upcoming renewal date, it must be made an X number of days before that renewal date. Otherwise, it will be moved over to the renewal after.

Note: For any of these 3 options, you can require your customers to interact with the advanced fields with the first checkbox and make it visible on your product pages with the second. Save when you are finished.

You can check your subscription product page to view how everything will look from your customers' end.

Order Date

Below is the checkout page of a sample Shopify order with a form field set (text) and filled out prior to purchase.

When a purchase is made with these fields filled out, they are stored as attributes in both Shopify and Appstle Subscriptions.

In Shopify, it will be visible in the Orders section. Click on the ID of an order to view the attributes (example is of Delivery Instructions).

In Appstle Subscriptions, it will be visible in the Subscriptions section. Click on the Subscription ID of a customer to be taken to the Merchant Portal.

From the portal, select the 3-dots icon on the far right of the subscription product and select Attributes.
