Documenting Bowel Movements and monitoring in Care Track

This article explains how to document bowel movements (BMs) as part of a care routine and record them electronically in Care Track. By default, bowel movement tracking is turned off for all residents and must be enabled individually in each resident’s chart.

Tracking Bowel Movements in Care Track

Step 1:

From the bottom of your screen, navigate to Care Track.

Step 2:

Open the individual resident’s Care Track chart.
You’ll have two options for documenting bowel movements.

Option 1 - PRN

Step 1:

Click “I gave extra care” in the bottom right of your screen.


Step 2:

Select 'Toileting.' Click 'Submit.'


Step 3:

Choose the bowel movement size, or select 'There was no bowel movement.'

Option 2 - Scheduled Care Routine

Step 1:

In the resident’s scheduled care, check the box next to 'Toileting.' Click 'Mark Done.'


Step 3:

Choose the effort level provided.


Step 4:

Then select the bowel movement size or indicate that none occurred.


Tracking Reminders

If a resident has bowel movement tracking enabled and no bowel movement has been logged in the past three days, a banner will automatically appear in Care Progress, moving that resident to the top of the list for review.


You can view historical Bowel Movement (BM) tracking at the bottom of the resident’s Care Log.


For users with access to Insights (such as Owners, Directors, and Nurses), BM tracking data is also available under the Care Track tab within Insights.
