When you are leaving your company group benefits plan, you and your family can choose to purchase individual life or health insurance within 30-60 days of your last date worked, with no medical requirement. After the 30-60 days (depending on the benefit), you may need to provide medical information in order to apply for coverage.

Health & Dental

If you were insured with Green Shield Canada then you may contact them directly and ask about their health and dental benefits called Prism Health and Dental Plan Options. You must do so within 60 days of your last day of work in order to bypass the medical questionnaire and/or exam.
