There are many reasons why your drug may not be covered.  

 - Your plan may cover generic drugs only
 - You are not the first payer on the plan (known as co-ordination of benefits)
 - Depending on your plan, this drug is not an approved drug under the Reformulary

It is best to call either your insurer or provider directly so that they can investigate further and give you an alternative, or ensure that the claim was adjudicated properly.
