It can be quite distressing to find out that your drug plan only covers you for a certain amount, and that you have to pay a large balance out-of-pocket that may not be affordable.

Beneplan has set up a Drug Advocacy program called the Compassionate Drug Assistance Program to help you.

The first step to accessing this service is to call Lucinda at 416-863-6718 X 240.
Alternatively, visit www.beneplan.ca and Click on Forms> Beneplan Forms Online> Drug Advocacy.  The form can also be found here:

You will then receive a further email where it will be necessary for you to electronically sign the form, and then once completed it will automatically be forwarded to our Drug Advocacy specialist who will contact you shortly thereafter.
