How to add a care plan for your client
A digital tool to guide the creation of effortless, holistic and person-centered care plans.

Our digital care planning feature allows you to digitise your care assessments which saves time but also allows you to create a task plan with ease. As the care plan is digital, it is easy to update so that you can deliver the best care possible to your client.

Carrying out a needs assessment allows for a greater understanding of an individual's physical, mental and emotional needs and exactly what type of support will be beneficial to them.

Below, we will show you how to create a care plan in birdie, how to download it into a document and how to link other relevant assessments to it.

Pro tip: If you have older care plans for the same client, you can upload them using the document uploader.

How to create a Care Plan on birdie

In the Agency Hub, go to the 'Clients' tab and select the client for who you would like to create a care plan. Click on 'Care Plan' on the left-hand sidebar and you will be taken to the Care Plan hub.

Add personal details

If it is a new client you can add personal details by clicking on the 'About me' tile and then clicking on the edit button 'Edit' to add personal information such as contact details, medical history and future planning. Click on 'Submit' when you are done.


Needs Assessment

Under the 'initial assessments' section, you will find 8 different categories for you to work with. In each category, you will be asked a number of questions about your care recipient so you can create a person-centred care plan which fully understands their needs.

We also have many additional assessments for you to fill in to ensure all the important details of your client's care are documented.

Completing a digital care plan on Birdie

When you click into a care plan you will see four areas that you will need to fill out: Needs assessment, Assessment summary and outcomes, Tasks and Risks/mitigations.


Assessment summary and outcomes

Click on the box under 'Assessment summary and outcomes'. This box is free text and is an opportunity for you to record and really personalise what your client would like to achieve with regards to their personal care. Click on 'Save changes' when you are done.

Tasks

Once you have recorded all of the outcomes, for this category you will be asked to complete 'Tasks' which will help the client achieve those outcomes. Start typing the task that you want to add and you will be shown a number of options.

Select the task you would like to add from the drop-down list and then follow these steps when the pop-up appears:

  1. Select the days on which the task should be completed

  2. Choose the time of day

  3. To save it, click on 'Add to task list'

The task will now appear in a list below the entry box and will then be shown automatically in the Task planner and on the carer app.


Risks and mitigations

If a client presents with a risk, which needs to be recorded, this can be done in this section. To add a risk or mitigation, click 'Add new risk', this is a free text field allowing you to add as much detail as needed. You can also add a level of risk and when you have finished simply click the 'Save' button.

You can navigate back to the Care Plan hub by clicking on 'Care Plan' on the Left-hand side menu. A green tick will appear on the tile which you have just completed with a date that will enable you to easily see if this section needs to be updated.

Note: Repeat these steps for each of the six key areas of care.


Upload documents for additional assessments

If you have any additional risk assessments or documents, which you would like to store in the Care Plan of your client then you can use the document uploader.

Click on the 'Documents' tile from the Care Plan hub page. Drag in any files which you would like to be uploaded to the care plan. These will then be accessible from this page in the future.


How to download a PDF of the Care Plan

Once you have completed the care plan, you can download it as a PDF and share it with your client. Access the Care Plan hub page and click on 'Download documents'. Here you can download them individually or all of them.
