How to download blank assessment templates from the Agency Hub [beta]

Leverage downloadable links to access blank assessment templates directly in birdie.

Birdie has introduced the ability for you to download a selection of blank assessment templates directly from within the Agency Hub - to further support you in capturing insights and assessing new care recipients as they come under your service.


What are the blank assessment templates?

We have created a series of blank templates containing Birdie's assessments, which can be accessed via downloadable links.

The downloadable assessment templates mirror the assessments that are found within the Agency Hub and contain exactly the same questions across needs and risks. The templates also include extra space for note-taking, as we appreciate that conversation can change pretty quickly.

You are able to download the different assessment templates via three links, including:

  • Initial Assessments - A zip file containing blank templates for:

    • 8 Initial Assessments and the About Me profile within Birdie. These assessments will be downloaded as individual PDF documents within the zip file.

    • A master PDF, which consolidates all assessments excluding the Mental Capacity, including both Initial and Additional Assessments, into one PDF document.

  • Additional Assessments - A zip file containing blank templates for:

    • 17 Additional Assessments within birdie. These assessments will be downloaded as individual PDF documents within the zip file.

    • A master PDF, which consolidates all assessments excluding the Mental Capacity, including both Initial and Additional Assessments, into one PDF document.

  • Auditing Documents A zip file containing blank templates for:

    • birdie's Auditing Documentation, including Service Reviews, Courtesy Calls, and Client Feedback.


How do I download and use these?

These will appear as individual links within the Care Plan tab of any individual Care Recipient. The links will be surfaced in three locations on that tab, including:

  • A link for the Initial Assessments templates will be surfaced under the 'Initial Assessments' section

  • A link for Additional Assessments templates will be surfaced under the 'Additional Assessments' section

When you click on a given link, a zipped folder will be downloaded containing the relevant files, as outlined above and depending on the link you have selected.


How to add notes & a signature to these assessments

You can select an assessment template or multiple templates, depending on your need. Once selected, you can open the PDF, where you will see the list of questions, places to record your answers to those questions, and areas for additional notes.

These templates can be used with a tablet and PDF editing software to enable them to be scribed on the top with a stylus, rather than typed into.

Please note: For Service Review, Client Feedback, and Mental Capacity / Best Interest assessments, the template will include a place for you to have the care recipient scribe a signature on top of the document, leveraging your pencil and tablet device.


How to navigate to a specific assessment within the master PDF

The master PDF contains blank templates for all Initial and Additional Assessments. To easily navigate between those assessments, you can leverage the links on the Cover Page.

Simply click on any of the links, as above, and you will be taken to that individual assessment template.

To navigate back to the cover page, simply click the link at the bottom of any page within the master PDF.


Where do I store the completed assessment templates?

You have the ability to upload these within the Documents section of a Care Recipient's Care Plan tab - or you can upload to your agency's specific storage solution, depending on your agency's protocol and process.

However, you should still input these insights and complete the digital assessments directly in the Agency Hub.


In what scenarios would these templates best be used?

We know that when speaking with a new care recipient and completing assessments across key areas of needs and risk, you need to be able to quickly jot notes down to ensure a complete capture of insights.

We also know that agencies would like to be able to document these notes down while following the best-practice structure and list of questions as outlined within Birdie's assessments.

This is especially useful in times of low or no signal, when a care provider cannot directly access the digital assessments and input directly within Birdie's Agency Hub, but would like to ensure all questions are asked and completed.
