In this article, we’ll walk you through how to assign forms to your athletes, and review the different form types so you can assign them with confidence.

Form Types at a Glance
General Forms - Assigned to individual athletes or teams for custom assessments, check-ins, or one-off surveys
Daily Logs - Assigned at the team level; appear daily under the athlete's assignments; ideal for recurring check-ins like wellness or training logs
Post-Workout / Performance Logs - Assigned at the team level and triggered automatically after workouts; capture performance metrics such as RPE and duration
Note: Daily Logs and Post-Workout Forms cannot be assigned to individual athletes.
Steps to Assign a Form
1. Access the Assignment Tab
To begin, navigate to the Assigned Members tab within the Form page.
2. Click Assign Form
3. Choose a Start Date
The start date determines when the form will be assigned and available for athletes to complete. This is particularly useful for pre-scheduling forms, such as a first-month check-in, which can be set up in advance to ensure athletes have the form when needed, without requiring you to manually remember to assign it.
Tip: Choose a start date that aligns with your athletes' schedule, so they can easily access and complete the form from both web and mobile.
4. Choose an End Date
The end date specifies when the form will no longer be available. If the form is not submitted by this date, it will be removed. This is especially helpful for time-sensitive forms such as holiday travel logs, which should be removed once they are no longer relevant.
Note: If no end date is set, the form will remain available indefinitely.
5. Assign Cadence
Determine how often a General Form should reappear for athletes. Review how to do this here.

As mentioned, these forms can be assigned to individual athletes. You can use them for tasks like personal check-ins or custom assessments.

Manual - Select individual athletes or teams directly.
2. QR Codes - Share a QR code with athletes for easy onboarding.
Notification Settings
You can choose whether to send a push notification to your athletes when the form is assigned.
Differing Start & End Dates
If multiple athletes are being assigned the same general form, you can set different start and end dates for each athlete. This allows you to tailor the assignment timeline based on individual needs. You can make this change from the assignment tab.
Helpful Workflows
Manage Assignment
At any time, you can add additional teams or individual members to an existing form assignment. Simply return to the Assign tab to manage the list of recipients and make any necessary updates.
Tracking Completion
Once the form has been assigned, you can track its completion status directly from the Assigned Members Tab. As athletes submit their forms, the status will automatically update to reflect whether the form has been completed or not.
