Using Activity Codes to Screen Employees for Covid-19
The screenshot below has examples of survey statements you might want to use. Instruct your employees that if they can answer yes to any of the first 4 statements, NOT to select one. Instead, close the app and contact a supervisor. Otherwise, choose the last one to verify none of the above to clock in.
