Several enhancements have been made across the 1Form experience to streamline data collection, improve clarity, and ensure regional compliance. The summary below outlines the updates for version 3.6, organized by section, for easy reference.

Business Section
Business Details
Added a new question: "Are you owned by a private equity?" with a Yes/No response. If Yes, an Effective Date field is displayed.
The file upload label for "Please specify your business type" is now dynamic based on the business type selected
Business Identification Numbers
Added a separate field for "Commercial And Government Entities (CAGE) Code (if applicable)"
Diversity Certifications
Reworded the parent question to "Does your company hold any officially recognized diversity certifications?"
Each certification now includes a question on whether it was issued at the state/province or national level. Selecting state/province reveals additional dropdowns for the location and issuing body. (Issuing body dropdown is US Only)
Added two new diversity certification options: Small Disadvantaged Business (SDB) and Lesbian, Gay, Bisexual, Transgender, Queer+ Owned Business Enterprise (GLBT). (US Only)
Insurance Policies
Renamed "Single Occurrence Limit" to "Per Occurrence Limit."
Construction Licenses/Certifications
Added "New York State Department of Labor Bureau of Public Work & Prevailing Wage Enforcement." (US Only)
Added a new question: "Which states are you licensed/registered to work in?" as a multi-select dropdown. (US Only)
Cybersecurity (New)
Added a new Cybersecurity subsection under Company Information with questions covering DoD contract involvement, CMMC compliance status, and CMMC certification level (Level 1, 2, or 3). (US Only)
Immigration and Customs Enforcement (ICE) Compliance (New)
Added a new ICE Compliance subsection under Staffing with seven Yes/No questions covering written immigration compliance policy, Form I-9 verification, internal audits, ICE audit response policies, worker training, and contingency plans. (US Only)
Legal
Added a new question under Financial: "Had any material delinquent tax obligations in the last 5 years".
Added a new question under General: "Been subject to any governmental investigation, enforcement action, or settlement relating to bribery, corruption, fraud, bid-rigging, false claims, prevailing wage violations, or similar misconduct".
Moved the company name change question to the General Legal section under the ownership/control question.
Finance Section
Most Recent Fiscal Year End Financial Statements
Added a new dropdown for "Accounting Basis of Financials" with options: Accrual basis, Cash basis, Income tax.
Added a new "Customer Deposits" field under Liabilities and Owners Equity, after Over Billings.
Employee Stock Ownership Plan (ESOP)
If the user has an ESOP, added a question for the last year ESOP ownership was increased. If within the last 5 years, additional questions appear covering ownership percentage change, corporate income tax status, financing method, remaining seller-financed debt, and current repurchase obligations.
Banking
Restructured the page to group Primary Bank questions together, followed by non-primary bank lines of credit.
Removed the institution field from Primary Bank Line of Credit cards (since it's all with the primary bank).
Added a Primary Bank Representative section with fields for name, title/position, and signed date.
Users can now specify multiple lines of credit from non-primary institutions, each with institution, currency, expiry/maturity date, authorized limit, current usage, and amount available (calculated).
Added Primary Bank - Covenant Details with questions on current and past-year covenant compliance. (Canada Only)
Added Primary Bank - Deposit Account Details (Last 12 Months) with currency, average account balance, and NSF activity questions. (Canada Only)
Added Primary Bank - Term Loan Details with fields for currency, expiry/maturity date, authorized limit, current usage, amount available, secured status, and repayment history. Users can add multiple term loans. (Canada Only)
Performance Bonding
Reorganized section order: Surety Provider Details → Surety Broker Details (new) → Authorized Bonding Capacity → Security Details (new) → Claims → Bonding Rates (new) → Surety Representative (new) → Bonded Projects.
Added an AM Best rating dropdown under Surety Provider Details.
Added a new Surety Broker Details section with broker company name and start year.
Added a new Bonding Rates section where users enter rates per $1,000 of bond for 50% and 100% coverage for Performance and Performance + Labour & Materials bond types.
Added a fast-track form bonds acknowledgment checkbox. (Canada Only)
Added a new Surety Representative section with name, title, signed date, and representative type (Surety Provider or Surety Broker with Power of Attorney).
Moved the attachment box to the top of the page with updated label referencing the bonding reference letter.
Health and Safety Section
Health and Safety Program
Added a conditional question when Hazard Communication Program is checked: "Does your Hazard Communication Program include current Safety Data Sheets (SDS) for all hazardous materials?"
Removed the "How often does your company hold a job site safety meeting?" question from this section.
Removed the Key Document section.
Moved "How often do you hold regular safety meetings for field supervisors?" from this section to Field Safety Practices. (US Only)
Safety Management and Planning (New)
Added a new question under Internal Policies with three radio button sub-questions covering employee training and toolbox talks, subcontractor pre-qualification based on safety performance, and job safety analyses/pre-task planning.
Field Leadership Safety Qualifications (New)
Added a new question asking whether foremen and superintendents hold current First Aid/CPR certifications.
Added an additional question asking whether supervisors complete OSHA 30/10 hour training or the STS Certificate. (US Only)
Field Safety Practices (New)
Added new questions covering safety pre-planning integration into project meetings, ongoing job site safety inspections (with follow-ups on inspection frequency and who follows up), high-risk work safety training, and competent person assignment for high-risk activities.
Moved "How often do you hold regular safety meetings for field supervisors?" here from its previous locations (Orientation Program for Canada, Health & Safety Program for US). This is a new question for the UK 1Form.
COR Certification
Added an Expiry Date field (date picker) when the user answers Yes to being COR certified. (Canada Only)
OSHA 300A/Workplace Illness & Injury
Updated the primary question to: "Do you have OSHA 300A forms for any of the 5 previous years?" (US Only)
If No, users are asked "Why can't you provide OSHA 300A forms?" and must still provide incident information for the previous 5 years.
If Yes, users complete data for the required years, or check "Cannot provide incident info" for any year with reasons.
ESG Section
Human Rights & Supply Chain Integrity (New)
Added a new subsection after Governance with two questions: whether the company has policies to prevent forced labor/human trafficking/modern slavery, and whether it has been subject to any related governmental investigation in the past 5 years. (US Only)
Compliance
Reworded existing questions (anti-bribery, employee training, third-party evaluation, government ownership, key individuals, and hospitality) for clarity. (US Only)
Added a new question: "Will your work involve direct interaction with any government agency or Government Official, including helping to obtain permits, licenses, approvals, inspections, customs, or other government services?" (US Only)
