When you put a patient in follow up from a Team Decision bucket, you have 3 options: Auto, Set, and Remind.
“Set” is your way to schedule a custom message to go out to a patient at a later date.
Click a patient's name and then click Set:

Clicking Set will open this window - type out the message that you want me to send to the patient, in the text box (you can also put someone in follow up from their Messaging Modal, which will open this window as well):
(TIP: use Templates to create premade messages so all you have to do is click the yellow icon under the text box and insert a premade message)
Options (above the message box):
Send Via: Choose whether you would like to send a Text or Email.
Responsible Party: Choose which responsible party you would like Ava to send the message to.
Details (under the message box):
Send Date: Choose the date that you want Ava to send the message on:
Time: Choose the time of day that you want Ava to send the message:
Category: Choose the Follow Up category that you're setting them for (New Lead, No Show, Pending, Recall):
