When you use my New Patient Tracker, you can enroll your patients automatically into a Forms sequence.
I'll send them texts with a link to fill out their HH before their consult.
They can fill out their forms right from their phones, and they are given the option to take a selfie, and front/back images of their Insurrance card as well π
If they forget, they can also fill them out from a device (like a tablet) in the office!
The great thing about it is you can save their forms as a PDF, but their info is saved right in my desk for up to 14 days after their consult!
I know, I know - I do it all! Don't you love consolidation? π
A blue checkmark in the Forms column means that patient submitted their forms.
Click the checkmark to open the PDF file to save or print the form!
