Easily build forms to collect information and signatures in DragonFly

DragonFly's form builder makes it easy to recreate existing forms or make new ones for your organization, whether you simply need a signature or need to create something more complex.

Build A Form

To begin building a form, navigate to the DragonFly Hub. Click 'New' then select 'Form'.

You'll see a new form. You can rename the form at the top where it says 'Untitled', then begin adding elements.

All elements - questions, signatures, information to be read - are added by selecting 'Add A Question'.

When you click this a blank short answer text question will appear. You can change the type of item by selecting the drop-down in the right corner and making your choice.

The options for questions are Short Answer, Long Answer, Multiple Choice, Checkboxes, Yes/No. You can also add non-question elements such as Heading and Formatted Text. Finally, you can add a Signature or Date. Below you can find information about each.


Question Elements

Short Answer

A Short Answer question is best for small amounts of text - names, phone numbers, etc. If an answer could be more than a few words or a single sentence, the Long Answer question is best.

When you add a Short Answer, type in both the question or prompt (in the above example, "Name") and a brief description, if desired.

Long Answer

The Long Answer question is useful when more information is needed. Examples could include requests for additional comments, or a full mailing address.

When you add a Long Answer, type in both the question or prompt (in the above example, "Home Address") and a brief description, if desired.

Multiple Choice

The Multiple Choice question is useful when you want to have closed-ended questions and a limited selection for answers, and you want only one selected.

If you want to give the option for selecting multiple items, the Checkboxes question is more appropriate.

When you add a Multiple Choice question, type in the question or prompt (in the above example, "Are you a student, a parent, or another family member?"), a brief description if desired, and all options you'd like to make available. You can reorganize items by clicking the up and down arrows, or remove them by clicking the trash icon to the right of the answer.

Checkboxes

Checkbox questions are useful when you want to offer multiple options and give the ability to select more than one. If you want to limit answers to only one item, the Multiple Choice question is more appropriate.

When you add a Checkbox question, add a question or prompt (in the above example, "Sports") and type a description if desired. Then type each item you want to make available as an option. You can reorganize items by clicking the up or down arrows by each item, or remove items by clicking the trash can icon.

Yes/No

If you simply need a yes or no for a question, the Yes/No question is the solution. Simply type in a question (in the above example, "Are you above the age of 18?") and a short description if desired.

Information Elements

Information elements in the form builder are useful when you want to give information for the form filler to read and respond to. If you simply need to give information (like a policy or waiver) and collect a signature, this is the block for you.

Heading

A Heading element is a large text element used to help organize your form. It is not intended for a large amount of text (no more than a title); if you need to put more text in, the Formatted Text is more appropriate.

Formatted Text

The Formatted Text element is useful for large amounts of text that needs to be read by the signer. An example could be an organization policy that needs to be read, or a photo release waiver (like in the below example).

This is also a useful field where you can paste in text from an existing document. In the below example, a photo release waiver is copied from a Google Doc and pasted into the Formatted Text field.

Please note: Only text can be copied and pasted. Other elements (such as logos and images) cannot be pasted into the form.

The Formatted Text element gives formatting options. Even when copy/pasting from another document, some work might be necessary to format the text to match what is desired.

Signature/Date Elements

Signature & Date Elements

The Signature and Date elements are useful for collecting an official signature and dating the form for compliance, record keeping, etc.

When adding the Signature or Date elements, include a title or prompt for the item and (if desired) a description for the item.
