How to make changes to my schedule?

We’ve made it easier for you to ask for any changes to your work schedule. Instead of phoning or emailing, you’ll now use our Schedule Change Form.

This means:

  • No more mixed-up messages.

  • No missed requests.

  • Everything written down clearly in one place.


Why we’re using the new form

1. Fewer mistakes:

Every request looks the same, so nothing gets lost or misunderstood.

2. Easier to track:

We can see how often changes are made and plan ahead better.

3. Fair for everyone:

We’ll follow company rules, like giving at least 2 weeks’ notice for non-urgent changes.

4. Everything recorded:

Every form is saved, showing what was requested, when, and by whom.


What you’ll need to fill in

When you open the form, you’ll be asked for:

  • Type of change (Extension, Early Leave, or Other)

  • Your full name

  • Client’s name

  • Current agreed date

  • New date you’d like (we’ll review this before confirming)

  • Reason for change

  • If you’re flexible (share your preferred days)

  • Whether you’ve spoken to the family

    Remember: Always talk to the office before discussing schedule changes with clients or families.

  • Any other notes


What happens next

Once you’ve submitted the form, our office team will:

  1. Review your request.

  2. Get back to you within 5 working days.

  3. Confirm if approved, or ask for more details if needed.


Using this form helps us keep things organised, fair, and stress-free — for you, the office team, and the families we support.
