📝 What is a reassessment?
A reassessment is a review of a care recipient’s current care needs when something has changed and the existing care plan may no longer fully reflect the support they need.
This helps ensure care remains safe, appropriate, and person-centred.
🔍 When should you request a reassessment?
Please complete a reassessment request if you notice significant changes in a care recipient’s:
🚿Personal Care
Increased support needed with washing or dressing
New continence issues
Skin concerns or pressure damage
Increased medication support needs
🚶 Mobility
Increased falls risk
Reduced mobility
Changes in transfers
New equipment requirements
🥗 Nutrition or Hydration
Reduced eating or drinking
Weight loss
Difficulty swallowing
Concerns around dehydration
🧠 Mental Health or Wellbeing
Increased confusion
Anxiety, distress, or low mood
Changes in sleep
Behavioural changes
Reduced capacity or communication difficulties

Deterioration in health
New diagnosis

Increased infection risk
Hospital admission or discharge

📋 How do I request a reassessment?
Please include as much factual information as possible, including:
What has changed
When it started
How often it happens
How it affects the care recipient or your ability to provide care safely
Avoid vague statements such as:
✖ “Client is worse.”
Instead use:
✔ “Client now requires assistance from two carers for transfers due to increased weakness and unsteadiness.”
⏳ What happens after I submit the form?
Step 1 – Acknowledgement
A Care Coordinator will acknowledge your request within 48 hours.
Step 2 – Review
A member of the Quality Assurance team will review the information provided.
Step 3 – Decision
We will decide whether:
A formal reassessment is needed
Care plans need updating
Monitoring or smaller changes are sufficient
Step 4 – Outcome
Possible outcomes include:
Care plan updates
Additional support recommendations
Equipment referrals
Change in care tier
No change required
You may be contacted for additional information during the process.
🚨 What if the concern is urgent?
If there is an immediate risk to safety or wellbeing:
Contact the office immediately
Follow emergency procedures where required
Contact emergency services if necessary
Do not wait for the reassessment form process in an emergency.
Examples:
Serious falls
Breathing difficulties
Safeguarding concerns
Severe deterioration
Unsafe moving and handling situations
Will raising concerns negatively affect me?
No. We encourage carers to report concerns promptly.
Requesting a reassessment demonstrates professionalism and supports safe, high-quality care. Your observations are extremely valuable because carers are often the first people to notice changes.
What happens if the care tier changes?
If a reassessment shows that care needs have increased or decreased significantly, the client’s care tier may be reviewed.
Any changes will be:
Discussed with the client or representative
Documented appropriately
Communicated clearly to all relevant parties
