How and when do I hand over to another carer?

When you are finishing your rota you will handover to another carer. The purpose of this is to have both carers in attendance to discuss the care package and ensure all relevant information is updated to date and shared effectively.

Handover structure:

  1. Handovers always take place at 1pm (unless confirmed otherwise)

  2. Both carers/family member in attendance

  3. Duration: 1 hour long

  4. Handover form to be completed by carer handing over and leaving the placement with support from new carer/ family member

Before the handover, the current carer will ensure:

  • Bed sheets are taken off the bed and are clean. Ensure a sufficient supply of fresh bed sheets is available for the next carer. If handing over to the family, bed sheets should be taken off and new sheets put on the bed.

  • The rubbish has been taken out.

  • All areas are clean and surfaces have been wiped down.

  • All personal belongings have been packed - carers are not allowed to leave anything at a client's home.

  • Fridge refresh - clear out-of-date food, label food (date opened, valid until, carer signature).

  • PPE stock - ensure at least 1 week of PPE equipment is on site.

Before the handover, the current carer will ensure:

  • Read the past 5 days' visit notes

  • Read and understood care plan

On the day for outgoing carer (in the following order):

  1. Introductions: If handing over to an edyn carer, current carer will make introductions between care recipients and a new carer. Please ensure the incoming carers IDs are worn.

  2. House: show incoming carer where everything is.

  3. COSHH assessment: Handover risk assessment of environment and ensure cleaning (hazardous substances) products are locked in a safe place.

  4. PPE stock: Ensure adequate stock is on site and show incoming carer the location. If further PPE is required, then please complete a PPE request form. You will be prompted to do this when you complete the handover form.

  5. Medication stock / MAR sheets: Show incoming carer the location of MARs, review MAR charts, check medication list which is given by pharmacy and check stock levels. If any medication is running low then please inform the pharmacy. Always follow medication training and NICE guidelines.

  6. Condition of client review: Handover of wellbeing of the client. Inform incoming carer of any new risks and any appointments that need attending. Note on the handover form if there is any update to the care plan, home risk assessment or risk assessment needed.

  7. Chart review: Together with the incoming carer, review the wellbeing charts, body map, food/fluid intake, bowel movements - if applicable.

  8. Financial review: Review the transaction sheet and remaining petty cash review (how much), document and sign.

  9. Keys - Handover of location of key safe or keys to the carer.

On the day for incoming carer (in the following order):

  1. Infection control: Please wash hands, take off shoes or wear shoe covers where appropriate and wear appropriate PPE.

  2. Introductions: Current carer will make introductions between care recipients and yourself. Please ensure your ID badge is worn when you enter the house.

  3. House: Current carer will walk you through the house to show you were everything is.

  4. Belongings: Drop your bags + inspect the carers room.

  5. Fridge and food review: Confirm items in fridge and food shopping list

  6. Medication stock / MAR sheets: Current carer will show you the location of MARs. Please review the MAR charts, check the medication list which is given by the pharmacy and check stock levels. If any medication is running low then please inform the pharmacy. Always follow medication training and NICE guidelines.

  7. Condition of client review: Current carer will give you a handover of the current wellbeing of the client. They will inform you of any new risks and any appointments that need attending.

  8. Chart review: Together with the current carer, please review the wellbeing charts, body map, food/fluid intake, bowel movements if applicable.

  9. Financial review: Please review the transaction sheet and remaining petty cash.

How do I access visit notes for previous days?

  1. Log in to your edyn.care account

  2. Click on the clients tab

  3. Select the client in question

  4. Click on the Visits tab on the right-hand side, next to the care plan tab

  5. Select the visit you'd like to review

What do I do if the incoming carer is late?

If the incoming carer is running late your care coordinator will inform you. We will request you stay at the property as long as you can to ensure the safety of the care recipient. Your care coordinator will work with you to ensure there is a plan in place.
