Answer:
There are two display settings for the labels and fields on your intake form.
Standard - Labels will display above the field on the left.
Side By Side - Labels will display beside the field on the left.
To change the layout setting do the following:
Go to Settings > Tools > Intake Forms
Click the pencil icon on the Intake Form you would like to make changes to
Click on the Styling tab
Find the Layout setting at the bottom of the list
Change the dropdown to the setting you would like
Click Save at the top-right of the screen
