Meals on Wheels: Offers condition-specific, medically tailored meals for individuals recently discharged from the hospital, those at high risk of hospitalization, or those requiring care coordination.
GA Foods: Provides either medically tailored meals or weekly grocery boxes, based on members' health conditions, personal preferences, and living situation. The program is designed for individuals with chronic conditions, those recently discharged, at high risk of hospitalization, or those with significant care coordination needs.
Lifespring Home Nutrition: Delivers medically tailored meals directly to the homes of members returning from the hospital or with specific dietary requirements. The service includes assistance in organizing freezer space to accommodate deliveries.
Project Foodbox by Sunterra: Offers nutrient-dense grocery boxes containing fresh fruits and vegetables, designed for individuals with qualifying health conditions, those recently discharged, or members at high risk of hospitalization and in need of extensive care coordination.
Bento: Supplies real groceries, including proteins, similar to what you would find in a typical grocery store. These groceries are tailored for members with chronic conditions, those recently discharged, at high risk of hospitalization, or with extensive care coordination needs.
CountyCare Medically Tailored Food Box Program:
Dion's Chicago Dream: Provides fresh produce boxes that can be used to prepare up to 14 meals, aimed at promoting wellness for members with chronic health conditions.
Chi-Fresh Kitchen: Offers heat-and-serve meals designed for general wellness or specifically low-fat, low-sodium meals to support patients with heart conditions. Meals are delivered directly to members' addresses.
