Vendor Assignment Process

CountyCare Medically Tailored Food Program:

  • Vendor assignment is based on claims-based algorithms. The following distribution occurs:

    • 80-90% of members receive food boxes.

    • 10-20% receive medically tailored meals (MTMs).

    • Typical program duration:

      • Food boxes: 3-6 months.

      • MTMs: 1.5-3 months.

CountyCare Emergency Meals Program:

  • Only pre-made frozen meals are available.

CalOptima Program:

  • Vendors are assigned based on chronic conditions and food box preferences noted in the risk assessment submitted by the member. If member would like to change their vendor and they have received four weeks of deliveries, please fill out a food support ticket.

  • Members may request a specific food vendor. RD’s must input this request in the “RD Recommendation” field in the risk assessment.

  • If a member does not to participate in the food portion of the program, they must cancel their enrollment directly with the vendor.

  • Members cannot change vendors within the first 4 weeks of being assigned to a new vendor. The only exceptions will be made for the following circumstances:

    • Religious exemption that can’t be fulfilled

    • Food allergies that their current vendor can’t guard against; and/or

    • Delivery challenges (e.g., the vendor is unable to deliver to the member’s location.)

Santa Clara Family Health Plan

  • Vendors are assigned based on chronic conditions and food box preferences noted in the risk assessment submitted by the member. If member would like to change their vendor, please fill out a ticket.

  • Members may request a specific food vendor. RD’s must input this request in the “RD Recommendation” field in the risk assessment.

Molina Health of IL- Maternal Health Pilot

  • MTM will be sent after the initial visit is completed


Advanced Health- HRSN

  • You will not have to screen members for food, they will already be screened for meals.

Cencal Vendors

  • For Medically Tailored Meals (MTM): We partner exclusively with Homestyle Direct. They provide fully prepared, clinician-approved meals that are ready to heat and eat.

  • For Medically Tailored Groceries (MTG): We partner exclusively with Bento. They provide credits for specific healthy grocery items that members can select.

Kaiser Permanente (KP)

  • Vendors are assigned based on their ability to meet the required Food Benefit Type and delivery timeline

  • For Priority orders (3-Day initial delivery), only vendors capable of priority MTM fulfillment will be assigned:

    • Mom’s Meals

    • Modify Health

    • Homestyle Direct

  • For Standard orders (6-Day initial delivery), vendors are filtered by the assigned Benefit Type:

    • If MTM:

      • Mom’s Meals

      • Modify Health

      • Homestyle Direct

      • Roots Food Groups

    • If Grocery Box:

      • Sunterra / Project Food Box

      • Food Health Collective

Honoring Member Preferences

Preferred Vendor: If a member expresses a preference to their Foodsmart RD during an assessment, that preference will be honored provided the vendor offers the assigned Benefit Type (MTM vs. Grocery).

No Preference / Incompatible Preference: If no preference is stated, or if the preferred vendor does not offer the required Benefit Type, the system will assign an eligible vendor using a round-robin model to ensure equitable distribution.
