Diet and Portion Size Accommodations

Updated over a year ago

CalOptima Program:

  • Dietary accommodations (such as halal, kosher, gluten-free, etc.) must be noted during the initial risk assessment. If a patient requests a dietary accommodation after their benefits have started, they can reach out directly to their assigned vendor.

  • Food box preferences are available to be requested during the member's risk assessment. Once the risk assessment is submitted and the referral is processed, the member is required to receive four weeks of deliveries before being eligible to change the food type, as per CalOptima policy.

  • Members can contact their food provider directly to request portion size adjustments.

  • GA Foods: 1-866-575-2772

  • Lifespring: 1-949-833-2920

  • Meals on Wheels: 714-229-3366

  • Project Food Box by Sunterra: 1-949-673-0783

  • Bento: 1-844-238-0718 / Support@gobento.com

CountyCare Medically Tailored Meals Plan:

  • Members are not able to request specific food preferences or change the type of food they receive.

  • The meals are tailored based on the member's responses in their enrollment form, which accounts for chronic conditions, allergies, and dietary needs.

  • Dietary accommodation requests should be directed as follows:

    • Dion's Chicago Dream: Members can contact the food provider directly at 312-509-4909.

    • Chi-Fresh: Reach out to Foodscripts@foodsmart.com for assistance in contacting the vendor.

CountyCare Emergency Meals Program:

  • Diet accommodations and food type preferences are not available, as this program only provides pre-made frozen meals.

If the patient is unsure of their assigned vendor, they can contact Foodscripts@foodsmart.com for further assistance.
