Program Goal: To help members with obesity lose weight and improve their overall cardiometabolic health with MNT and meal planning, grocery lists.

Target Population: Adults (18+) with a BMI of 30 or greater.

Registration Errors

We’re seeing an uptick in registration issues for Elevance Cardiometabolic members. Initial outreach from Elevance mistakenly directed members to register via Foodsmart.com using their insurance card ID. This process causes errors when members try to add a payment method.

Important: Members must register using their personalized link. Do not have members register through Foodsmart.com.

If you encounter a member who registered incorrectly:

  • Before the visit: Submit an eligibility ticket.

  • At the visit: Ask the member to call Foodsmart Member Support at 415-800-2311 for help and cancel the visit.

  • After the visit (if completed): Follow the same process above.

We’re also seeing cases where members mistakenly used the Anthem Pilot (Employer) plan. This plan is no longer active. Please do not schedule or complete appointments for members with this payment method. These profiles are being disabled and the members are being contacted to re-register correctly. To spot this error, check the insurance plan—if it shows as Anthem Pilot, it’s an invalid registration.

Extra Information:

This plan is offered through Elevance in partnership with Vida. Patients will first meet with a Vida health coach dietitian, whose focus is on lifestyle coaching in areas such as sleep, stress, physical activity, and overall wellness. Your visit will build on this foundation, providing individualized MNT nutrition care and clinical support.

Here is some sample scripting around how to introduce yourselves.

Hi, I’m (your name), a registered dietitian with Foodsmart. I’m here to support you with personalized nutrition guidance and help you reach your health goals. I understand you recently met with a Vida health coach . I’d love to hear a bit about that experience so we can build on what you’ve already started.

Prompting Questions:

These are some examples of questions to probe on what the patient has learned so far.

  • Can you tell me what you and your Vida health coach have talked about so far?

  • What kinds of things have you been focusing on with your coach? Sleep, movement, or general wellness?

  • Were there any goals or habits you started working on together?

  • What did you find most helpful or interesting from that visit?

  • Is there anything you wish we could go deeper into today that didn’t get covered yet?

If a patient has questions about seeing two providers (a health coach dietitian at Vida and a Foodsmart dietitian); here’s what you can say.

If the patient says: “I don’t know why I’m here.”

I totally understand. It can be confusing with all the different programs and providers. You may have met with a Vida health coach dietitian recently. That visit focuses more on general wellness topics like sleep, movement, and lifestyle habits.

My role today is to support you more specifically with your nutrition, whether that’s managing a health condition, improving your eating habits, or helping you reach your personal goals. I’m here to make this time useful for you. Would it be okay if I asked a few questions to better understand where you’re at?

If the patient says: “I already met with a dietitian at Vida.”

Yes! That’s great! The Vida team usually focuses on lifestyle coaching and big-picture wellness. I’m here to build on that and dive more into the nutrition side of things.

I’d love to know what you’ve already worked on so I don’t repeat anything. Is there anything that stood out from your last visit or anything you’ve started doing differently since then?

All of these patients will have a BMI >30 so you may find the Healthy Weight Program helpful. However, feel free to use any of the Foodsmart programs that are relevant to your patient.
