Submitting your Annual Questionnaire

Here is a guide for nonprofits on how to complete an annual questionnaire required for filing renewals.

Receiving your questionnaire

  • Your compliance specialist will email you a link to your annual questionnaire. Click the link to open the questionnaire.

  • Initially, you will have to enter your first name, last name and email.

  • Next, acknowledge your Compliance Specialist’s message.

Filling out your questionnaire

  • Once you have clicked on "Acknowledge," you will see three tabs to the questionnaire:

    • Nonprofit information

    • Individual state information

    • Fundraising agreements

  • Additionally, you will be prompted to review and/or include information about your organization, including:

    • General information

    • Organization information

    • Addresses

    • Organization contacts

    • Extra details

    • Compliance and legal history

    • Officers and Board Members

    • 5 Highest paid employees

    • List of banks

    • List of accountants

    • Government grants

Saving your questionnaire

  • The form autosaves every 60 seconds, but you may also press the Save button on the top-right corner of the questionnaire.

Submitting your questionnaire

  • When you are ready to submit your questionnaire, you may click on Save and complete at the bottom of the "Fundraising agreements" tab.

  • Once you submit your form, it will be locked for further review. To make further edits, you’ll need to contact your Compliance Specialist.

Note: All form fields are required to submit the questionnaire.

If a question does not apply to your organization, please enter “N/A.” If you try to submit the form before completing all required fields, you will be prompted to finish the missing information. When this occurs, the tabs at the top of the form will indicate how many fields still need to be completed on each tab. Please review each tab, fill in any remaining fields, and then attempt to submit the form again.

  • You will then be notified on the next page that the questionnaire has been completed.

  • To continue monitoring the progress of your renewals, sign up for an account on Change by clicking on Confirm your email.

  • Check your email inbox for a confirmation email from Change. Click Set password to finish setting up your account.
