Testosterone Replacement Therapy (TRT) is a medical treatment used to restore healthy testosterone levels in men who have been clinically diagnosed with low testosterone (also known as hypogonadism).

TRT is intended for men who experience ongoing symptoms related to low testosterone and have blood test results confirming deficiency.

What is Testosterone?

Testosterone is the primary male sex hormone responsible for:

  • Sex drive (libido)

  • Erectile function

  • Muscle mass and strength

  • Bone density

  • Mood regulation

  • Energy and motivation

  • Red blood cell production

  • Mental clarity and cognitive function

Testosterone levels naturally decline with age, starting around age 30, but some men experience a more significant drop leading to symptoms that affect daily life and long-term health.

Symptoms of Low Testosterone

Common symptoms include:

  • Low energy or chronic fatigue

  • Reduced libido or erectile issues

  • Loss of muscle mass or strength

  • Difficulty losing fat

  • Depressed mood or irritability

  • Poor concentration or memory

  • Lack of motivation

  • Poor sleep

  • Reduced sense of well-being

If these symptoms persist and are confirmed by a blood test showing suboptimal testosterone levels, TRT may be appropriate.

Why TRT Is Prescribed

TRT is prescribed to restore testosterone to healthy physiological levels—within the upper half of the reference range—aiming to eliminate symptoms and improve overall physical, mental, and sexual health.

TRT is not about “high” testosterone. The goal is symptom resolution with the lowest effective dose to maintain healthy levels and minimise side effects.

Forms of Testosterone Treatment

Our doctors prescribe testosterone in the following forms:

  • Injections (most common and effective)

    • Usually administered intramuscularly (into the muscle) 2–3x per week for stable levels

  • Transdermal (cream or gel)

    • Used for men with needle phobia or special circumstances

The most suitable option depends on your preference, lifestyle, and medical history. Your doctor will guide you based on clinical need.

Supporting Medications

In some cases, doctors may prescribe additional medications to:

  • Manage side effects (e.g., estrogen-related symptoms)

  • Preserve fertility (if actively trying to conceive)

  • Optimize outcomes (e.g., adjust estradiol or DHT balance)

These are only used if clinically indicated and are prescribed based on follow-up blood work and symptom reviews.

What to Expect on Treatment

  • Weeks 1–4: Subtle improvements in energy, sleep, and mood

  • Weeks 4–6: Libido may increase; muscle strength may begin improving

  • Weeks 6–8: Most patients report improved energy, sex drive, and mental sharpness

  • Week 8: First follow-up blood test to assess testosterone and estradiol levels

  • Weeks 8–16: Continued improvements in body composition, mood, and recovery

  • Weeks 16–26+: Stabilisation and long-term symptom resolution

Improvement is gradual and accumulates over time, especially when combined with lifestyle changes (e.g., nutrition, training, sleep).

Who Is Not a Candidate?

TRT is not prescribed if:

  • Blood tests are in the normal range

  • Symptoms are due to other causes (e.g., depression, poor sleep, high stress)

  • There are contraindications such as prostate cancer or untreated sleep apnea

  • You’re trying to conceive in the next 6–12 months (unless fertility medications are added)

Your doctor will determine eligibility based on clinical history, symptom profile, and pathology.

Is TRT Safe Long Term?

When medically supervised with regular blood testing, TRT is considered safe and effective. Your doctor will monitor key markers such as:

  • Testosterone (total & free)

  • Estradiol

  • Hematocrit & hemoglobin

  • Lipids

  • Liver function

  • PSA (prostate-specific antigen)

Follow-up testing ensures any risks are mitigated and dosages remain optimal.
