Viewing a questionnaire
Third-party vendors are an essential part of any organization's ecosystem, but they can sometimes pose serious risks. To mitigate these risks (or to avoid them altogether), organizations rely on vendor questionnaires.
Within Hyperproof, you can create, import, edit, and send questionnaires directly to vendors via the vendor module. When a vendor submits a response to a questionnaire, the questionnaire's owner receives an email notification.
A submitted questionnaire can be resent at any time if you need more information from the vendor.

Navigating and monitoring questionnaires
Within the vendor module, you can create and send questionnaires to your vendors in Hyperproof. The benefit of this is that you can leverage questionnaires to assess a vendor's risk to your organization. The responses for these questionnaires are sent right to Hyperproof to view.
In this video tutorial, we'll go over accessing a created questionnaire. We'll also cover how to adjust the settings and how to view responses from sent questionnaires.
The tutorial below is shown in the administrator role with organizational permission as a manager in Hyperproof. If you are in another role in Hyperproof, or have a different permission, you may not have access to some of these areas shown, or they may be greyed out.
With the latest update, facepiles are now available on questionnaires, providing a more interactive and engaging experience. Please note that this video tutorial was created before this update and may not reflect this new feature.

Sending a questionnaire to a vendor
Sending a questionnaire to a vendor
From the left menu, select Vendors.
Select the Questionnaires tab.
Select the questionnaire you want to send.
In the right pane, click Send.
The Send questionnaire window opens.
From the Vendor drop-down menu, select the vendor you want to send the questionnaire to.
From the Respondent drop-down menu, select the individual or individuals to whom you want to send the questionnaire. Note that this can either be the vendor owner OR the vendor contact.
Tip: To send the questionnaire to all contacts and/or owners, select the Select all respondents checkbox.
Optionally, enter a brief note for the respondent or respondents.
Choose any updated recipients to whom you'd like to send a notification once the questionnaire is submitted. They will then be able to view the questionnaire responses.
Click Send, and then click Done.
The questionnaire is sent.
Sending a questionnaire
You can choose to send a questionnaire to a single respondent or multiple respondents. Let's go over how to send out a questionnaire to not only a single vendor but also multiple vendors at a time.
By design, the Attachment icon (paperclip) is present on all questions, even those that are not 'proof upload' questions. Additionally, there are no restrictions on file types that can be uploaded as proof. See Viewing proof for supported file types.
All file uploads are scanned and Hyperproof's Operations team is notified of any infected files. Text input is sanitized for malicious HTML/Script.
Note, questionnaires will be coming from noreply@hyperproof.io. It's important to inform your vendors so they are aware to look out for this email and check in case they go to spam.

From the left navigation menu, select Vendors.
Select the vendors that correspond with the questionnaire you want to send.
Click Send.
From the Questionnaire drop-down menu, select the questionnaire you want to send.
From the Respondentdrop-down menu, select an individual to send the questionnaire. Note that this can either be the vendor owner OR the vendor contact.
Optionally, enter a brief note for the respondent or respondents.
Choose any updated recipients to whom you'd like to send a notification once the questionnaire is submitted. They will then be able to view the questionnaire responses.
Click Review and then click Send.
Click Done.
Canceling a questionnaire
A questionnaire can be canceled at any time if it has not been submitted. Suppose you cancel a questionnaire without notifying its recipient. In that case, when they click the Launch questionnaire button in their email, they’ll receive a message stating that the questionnaire is no longer active.

Select the questionnaire you want to cancel.
In the right pane, expand Responses, and then do one of the following:
Click the Sent icon if the questionnaire has not been started
Click the In Progress icon if the questionnaire has already been started
Mouse over the questionnaire, and then click Cancel
Click Confirm. The questionnaire will be canceled
