Creating cover page templates

Any fax within the Collaborative Health Record (CHR) is consolidated as a PDF before it is sent out. You can add a cover page when you send it out (see Adding a cover page to a fax).

When building your template, you must upload a base PDF document that serves as the background for your cover page.

๐Ÿ“Œ Note: You must have the Templates: Cover Page permission to create or modify cover page templates. See User permissions.

Cover page templates can contain text boxes, check boxes, and signatures that you can configure to automatically populate with patient data or that you manually enter when sending a fax.

These fields can also be configured in such a way that when you add information to a text box or checkbox, it can move back into the patient chart.

You can also create autofill cover page templates. By using these templates, you can complete cover pages with data points pre-populating directly into the cover page before you have even opened it. See Creating autofill cover page templates for more details.

๐Ÿ’ก Tip: You can view your archived, active or all cover pages you have created in the past by clicking Active > select All, Active or Archived next to Cover Page Templates.

Steps

  1. From the main toolbar, click Settings > Templates > Cover Page.

  2. Click Add Cover Page.
    A window New Cover Page Template appears.

  3. Under Template Name, type the name of the cover page.

  4. Under Base Document, click the paperclip icon and select a PDF document from your personal device.

    ๐Ÿ“Œ Note: Ignore the Reconcilable checkbox - not applicable for fax cover pages.

  5. Click Save.

    Wait a few seconds for the cover page to appear in the list of cover page templates.

    โ€‹๐Ÿ’ก Tip: If the red In progress sign next to your new template has not disappeared within 1-2 minutes, refresh your browser.

  6. Click on a cover page template you want to configure.
    The cover page template window appears.

    โ€‹๐Ÿ’ก Tips:

    • Click the expand button to maximise or condense the template window.

    • Click the magnifying glass to zoom in (plus) / out (minus) of a specific section of the template. This allows for accurate configuration.

    • Click Template Setting to edit the name and base document of a created cover page template.

  7. Add one or more of the following to your cover page template:

    • To reposition the text box, checkbox or signature box, click and drag it to the desired location.

  8. Once all fields are created, click Save.

  9. Refresh your browser to apply the changes to your account.
