The TELUS Collaborative Health Record (CHR) forms are electronic versions of the standard forms your clinic uses. You use these electronic forms while in a patient's chart. See Using electronic forms.

Marking conditions enable you to specify circumstances when a checkmark is auto-selected based on data in the CHR. The checkbox is first mapped to a patient or referral data field, and then you set the data that must be present for the checkmark to be checked.

For example; you can configure a pair of checkboxes for male and female to be selected based on the patient's sex set in their demographics. The checkbox is mapped to Patient Data as follows:

  • Category = Demographics

  • Data Property = Sex&Gender

  • Visible Data = Sex

  • Marking Conditions = Equal Male (for the male checkbox) and Equal Female (for the female checkbox).

Before you can add marking conditions to your form to auto fill, you must first create your template. See Creating cover page templates and Creating form templates.

Steps

  1. Click the area in your form or cover page template where you would like to add the variable. A new window appears.

  2. Click Check Box. A new window appears.

  3. Apply any configuration needed (see Adding checkboxes to form or cover page templates).

  4. Click Config Marking Conditions. The Configurate Auto Marking Conditions window opens.

  5. Click +Add Condition.

  6. Choose to mark when Any or All conditions are met.

    📌 Note: All conditions is used when you have an exact list from patient data that you want to auto-populate into the form.

  7. From the list, choose Equal or Not Equal. Most of the time you will use Equal.

    📌 Note: If equal is selected, the box will be selected if the patient data is equal to the described condition.

  8. In the empty field, type the data you want to match (equal) or not match (not equal). This defines what data needs to be present in the patient's chart for the checkmark to be checked. In the example above, the match is Male or Female.

    📌 Note: You must type the data exactly as it appears in the CHR (i.e. Enter Male not M or male).

    💡 Tip: We recommend you copy and paste from the patient data section in the patient chart.

  9. If required click +Add Condition to include multiple conditions to your rule. For example, if a checkmark asks if the patient has an allergy to anticonvulsants, include conditions for all specific anticonvulsant allergies.

  10. Click Save.
