Each federal state has its own form, which you can easily download from your tax office's website.
Here's how:
Select your federal state from the list.
Download the mandate template.
Fill out the mandate completely.
Print out the form and sign it.
Send the signed form to your tax office.
Some tax offices already offer the option to fill out an online form and/or send it via email. For more information, please visit the website of your relevant tax office.
Please note that you must grant a SEPA Direct Debit mandate for each tax number.
