Create and customize forms using jmc's stock form library in Form Designer

Get a head start on custom forms such as student forms, attendance letters, and report cards using our handy templates.

We understand that creating forms from scratch can be time-consuming and require trial and error. That's why we are giving you a head start to make things easier for you. We've created a library of stock forms in jmc's Form Designer, loaded with "DEMO" templates such as attendance, registration, health, and emergency forms, custom report cards, standard and benchmark report cards, as well as previously created forms. Save time and effort by choosing a template that suits your needs, duplicating it as a new form, customizing it to your requirements, and saving it as your own in just a few minutes.

To begin, simply log in to jmc Office and navigate to any of the form designer pages:

  • Form Designer > Student Information

  • Form Designer > Report Cards

  • Form Designer > Standards and Benchmark Report Cards

Step One: Click the "New Form" button to choose a template to use as the foundation for your form.

Step Two: Select a form template from the "Select Form Template" drop-down list to customize your form. You have three options:

  1. "DEMO": a pre-populated template to jumpstart your form creation process

  2. "Blank Form": an empty form for you to design from scratch according to your needs

  3. Previously created form: a form you've previously created and use it as a template for your new form

Step Three: Enter a name for your new form in the "Enter Form Name" field to give your form a unique name and distinguish it from the template.

Step Four: Click the "Save" button to save your newly created form to your Stock Form library, or click the "Cancel" button to exit.

Step Five: Customize your form by adding logos, custom fields, criteria for absences/tardiness, student information, health fields, student photos, and more.

Step Six: Click the "Save" button to save your customized form.

A few helpful tips:

  • Once you've customized and saved your new form, it will be available in the "Form" drop-down list for future use and modifications, and added as a template in your form library.

  • No longer need a form in your library? Click the "Delete Form" button, choose the form from the "Select Form Template" drop-down menu, and then confirm the deletion by clicking the "Delete Form" button. Just keep in mind, if you think you might need this form in the future, steer clear of the delete button.
