How To: Upload Questions for Forms

Global Admins and Managers can upload question lists to create custom forms. They can also allow lower-level regional and company admins to create custom forms using these same question lists.

To access the setup area, go to the "Companies" list and click on the parent company you'd like to add questions to. Next, click on the "Forms" tab on the right.

Next, click on the 'Questions List' tab in the center of the screen

There are two ways to add new questions to a company: through a CSV upload or individually by writing the question and selecting the question type. Both methods are explained below:

CSV Upload


To upload a list of questions a parent company, its a simple process of filling out a simple 2 column CSV, then uploading it to the company by clicking here:

By clicking on that button, you'll be able to download the template from the popup which consists of these 2 columns:

Question

Question Type

Question Types:

  • Yes / No

  • Open Field

  • Date and Time

  • File Upload

  • Signature

Example:

Is this a Staff Injury?

Yes / No

What is your Full Name?

Open Field

Current Date and Time

Date and Time

ID Upload

File Upload

Staff Signature

Signature

Individual Addition


You can easily add questions at the individual level. While on the list of questions, simply type the question, select the question type, and click "Save"
