What is the Chronic Disease Management Plan?

The Chronic Disease Management Plan (formerly known as the EPC) is a Commonwealth government initiative that lets people with chronic ailments or injuries access to professional services with the benefits of a Medicare Rebate so they can better manage their condition. It allows you to utilise the services of an Allied Health Professional up to five times within a calendar year while receiving a rebate through Medicare.
