Yes — you control both how the quiz appears on a page and which pages it appears on.
Choose how the quiz appears
The quiz itself always opens as a modal overlay. In the quiz builder's Placement tab you choose the trigger shoppers see on the page:
Sticky button — a floating button pinned to the page. Choose from 9 positions (top, middle, or bottom × left, center, or right) and whether it stays fixed while scrolling.
Button — a button placed within your page content; set it to fill the available width or size to its text.
Banner — a full-width banner with a heading, subheading, and call-to-action button.
Announcement bar — a slim bar at the top or bottom of the page.

Position the widget block in your theme
The quiz is delivered through a PerfectMatch block that you add in the Shopify theme editor. Because it uses Shopify's native section and block system, you can place it on any page template and reorder it like any other block. The builder's Publish tab shows a checklist of which page templates already have the block added.
Choose which pages show the quiz
In the builder you can also set visibility rules for each quiz:
All pages — the quiz shows wherever the block is present
Specific — only on the collections and/or products you pick
Quiz products — only on pages of products that are recommended in the quiz
