Logging Time Spent on CCM

Use Phamily to log time spent on care management activities in one place.

Phamily automatically records the time you spend messaging patients, but often care management involves activities that you are already doing in clinic, like chart reviews, phone calls, referrals, refills, and more. Logging time spent on all CCM activities for all enrolled patients (whether they have a SMS capable phone or not) ensures that you can successfully bill for your work.

1. Log in to Phamily and click Enrolled at the top of the page.

2. Phamily’s Quick Filters allow you to filter the patient list and prioritize your work. You can view by care time logged, engagement, and more.

You can also filter by Provider, Care Manager, or Patient Status. Click any patient on this list to view their profile and conversation history.

Track time spent on CCM as a Note

1. Log in to Phamily and search for the patient you wish to update. Click the patient’s name to view their profile. Alternatively you can click on the patient’s name from your organization’s list of Enrolled patients.

2. Click the Add Note icon next to the patient’s name.

3. Choose an option from the dropdown that appears. Phamily has options for common care management activities, or you can create your own note by clicking New Note…

4. Add context about the care management activities performed in the window that appears. You can back-date activities if needed. If you spoke to the Patient on the phone, check the Spoke to Patient on phone box. Hint: if you are already leaving a note about this activity in your EMR, just copy and paste into Phamily.

5. When you are finished, click SUBMIT. The patient’s logged CCM time will update automatically.
