What is “My Day”?
The "My Day" dashboard is a landing page designed and organized to support Care Managers with a workflow that prioritizes daily activities and tasks proven to successfully manage a panel of 200-300 patients.
With "My Day" you can handle the most relevant conversations, get a reminder to send check-ins, complete due/overdue tasks, create care plans, and review patients who have not signed PHI Authorization.
Handle Flagged/Important Conversations
At the start of every day, you should handle the most important conversations first. This dashboard will show you all conversations that are flagged, escalated, or potentially important.
Note: “Potentially Important” means that Phamily found something in the latest message that suggests there is something you should look at.
To handle conversations:
Open each conversation, as you would in the inbox
Reply to the patient, close that conversation, and work your way through the list
Pro Tip: Want to see your oldest messages instead of your newest? Sort by “Waiting Longest” instead of "Newest"

Once you respond to a conversation it will leave this page and can be found in the "Tracking" tab in your inbox.
Check-in with patients
After responding to patients in Handle Flagged/Important messages, look for which patients you should send an outreach to by sending them a check-in.
Follow the button to “Go to the Check-ins Page” and send a check-in.
Pro Tip: If you'd like to review the patients first, you can click the button at the bottom to "See Reachable Patients"
Complete Tasks
After sending Check-ins, take a look at tasks that should be handled today.
Review each task and either:
Complete them as necessary
Reschedule for a different day

You can remove yourself from a task. Just make sure to assign it to someone else so it doesn’t get lost.

You can click on the patient’s name in the task. This will open a new tab with the patient’s chart.
Create care plans
After taking care of all your patients immediate needs, it's time to create care plans.
To bill for a patient, you must create a care plan for them. We’ll show you the top 5 patients you should create a care plan for in order to ensure you’re on track to get them qualified.
Click on the “+ Care Plan” button to go to the care plan page and create one for them!
Pro Tip: To only see patients who have given PHI Authorization, click to toggle "PHI Consented Only" on.

To see patients you completed care plans for today, click “Extend completed patient list”.
Review for PHI consents
To bill for Connected Care services you will need patient consent. The next step is to review for patients that Phamily thinks are the highest priority for consent.
Click on the “+ Review” in order to open the patient chart
Review for consent and update the chart accordingly
Pro Tip: If you are interested in checking in with a lot of patients at once, reach out to us at support@phamily.com, or by clicking the question mark "?" in the upper right hand corner to ask about a consent campaign.
