📄 Instructions for Using the RX Destroyer

Learn how to safely dispose of DEA-controlled drugs with RX Destroyer and meet compliance using DEA Form 41 and Boomerang Pak return system.

The RX Destroyer is intended for DEA-controlled substances that cannot be returned for credit.

These drugs must be rendered non-retrievable in accordance with DEA regulations and must not be disposed of in the sewer.

  1. Load Medications

    • Place the non-creditable DEA-controlled medications into the RX Destroyer container.

    • Ensure that only approved substances are placed in the container. Examples include:

      • Chloral hydrate

      • Fentanyl sublingual spray

      • Phenobarbital

      • Testosterone gels or solutions

      • Valium injectable or gel

      • Sedatives, analgesics, anticonvulsants, and anti-anxiety medications

  2. Complete DEA Form 41

    • This form is required by the DEA to document the destruction of controlled substances.

  3. DEA Form 41 Instructions:

    • Section A: Fill in your complete address, contact information, phone number, and DEA registration number.

    • Section B (1): List the inventory (medications) you are destroying.

    • Section B (2): Leave this blank (do not complete).

    • Section C: Indicate the Method of Destruction as RX Destroyer.

    • Section D: Ensure that two authorized employees sign as witnesses to the destruction.


📦 Returning the RX Destroyer for Disposal

Once the RX Destroyer container is sealed and the DEA Form 41 is completed:

  1. Place the sealed RX Destroyer container into an RP Returns "Boomerang Pak":

    • This is the designated disposal return packaging system.
