Locate the prepaid return form included in the system box.
Fill in the following information clearly:

Contact information

📬 Step 3: Prepare for Shipment
Place the sealed container and completed return form back into the original shipping box.
Verify the prepaid shipping label is still attached and readable.
Drop off the box at any FedEx Ground location, or schedule a pickup directly through FedEx.
