ReturnLogic's questionnaires allows you to add content such as photos, links to further information, and individual questions to the return portal process. Thanks to our logic engine, you are able to decide when the questionnaires show up - whether for specific RMA types, specific items, you're able to customize their visibility.
Questionnaires will appear after both the return reason and return type have been chosen. For exchanges, this will pop up prior to browsing the catalog to choose an exchange item.
Setting Up Your Questionnaire
Head into your settings to Customize > Questionnaires.
You can have as many individual questionnaires as you like, there's no limit.
Parts of the Questionnaire
Name
Internal-only, name your questionnaire something descriptive, especially if you're going to have more than one.
Blocks
Input
The question itself, composed of both the Display Name (shown to shoppers on the field) and the Metafield (internal reference for reports). Each input option can be marked as required or left optional, and each questionnaire can have multiple input fields.
Content
Everything but the input field itself - you can add further text (like a title), links to troubleshooting documents, images. You can add multiple content blocks to each questionnaire. Content blocks help give shoppers an idea of what the questionnaire actually is, what they're filling out.
Metafields
The internal reference point for Input fields. There can only be one metafield per input and each metafield can only be used once in a questionnaire.
Logic Engine - When the Questionnaire Appears
You can set logic based on RMA type, return reason, order date, tags, and other criteria. This will allow you to granularly set exactly when a questionnaire will appear to your shoppers. By default, no questionnaire is selected.

Can you embed surveys into content blocks?
That is not currently an option. You can link to further surveys, however.
Can you add code in the content block?
No, however any Custom CSS in your settings will apply to content blocks, such as fonts, colors, etc.
