How To Remind Patients To Fill Out Their Form

Do you want to include the completion status of forms to ensure you have a smooth office experience? Conditional Logic has you covered!

Conditional Logic makes it so Aloha checks for the completion status of a form. It will allow you to include in your appointment reminders, to remind a patient to fill out a form or thank them for completing their form. It also allows you to customize verbiage depending on status. For example: If a patient has not completed their form, you can add in wording to the reminder to let them know to please complete the form before their visit.

To Set Up Conditional Logic in a Template:

  1. Go to Campaigns > Show Templates > Select the Template you want to add the Conditional Tag to or create a new one.

  2. Select Appointment Reminder next to Type:

  3. Next to Logic: select Form Status and click Insert, you can add in logic for a completed form and one that is for a non completed form into the same reminder.

    Don't forget to add the contact verification token at the end of the link to the form, without this a form wont link to the patient.

  1. Below is an example template with the recommended format.

    1. Text (Forms):

      {FORM_COMP}Thank you for completing your intake forms. See you at ~Appointment.Time~ on ~Appointment.Date3~. {/FORM_COMP}{FORM_INCOMP} Friendly reminder of your appointment at ~Appointment.Time~ on ~Appointment.Date3~. Please complete your intake here: getaloha.com/np-intake?~Contact.VerificationToken~{/FORM_INCOMP}

Now your template is ready to be added as a step to your Campaigns! You must first CREATE AN APPOINTMENT CAMPAIGN before the step can be added. Once your Appointment Campaign is created, follow the steps below to add your new template to a Campaign.

​How to Add Steps to a Campaign:

  1. Go to the CAMPAIGNS tab > SHOW CAMPAIGNS page

  2. Click on your desired campaign

  3. From the Edit Campaign page, click +Add Step

  4. Enter an easily identifiable name for the step (e.g. Family Appointment/Forms Reminder (Forms Complete/Incomplete))

  5. Use the dropdown menu next to Action to select the action to be performed (e.g. Send a Text Message or Send an Email)

  6. Use the new dropdown menu that appears for Action to select the specific template to be triggered.

  7. Next, specify When the step should be triggered: Enter the amount of time (e.g. 0, 1, 5, 15, 30)

  8. Choose the unit of time from the dropdown (e.g. Minutes, Hours, Days)

  9. Enter when you would like the step to be executed (Before, After or Into Campaign). For Example: The step will execute “30” “Minutes” “ Into Campaign”

  10. If you are using the conditional logic to check for form completion, select the form Aloha should be checking the completion status for from the drop-down menu next to “For logic conditional tags use”.

  11. Click Save Campaign Step to save.

  12. Repeat until each step needed in your campaign has been added
